T00163 Request for Waiver of Requirement to be Present on Last Day of Service

Instructions:

Welcome to the World Bank Group HR Forms Instructions page.

This form filler tool is designed to assist you in electronically entering your data and printing a paper form with a unique barcode corresponding to your entered data. Once you've completed your answers and printed the form, you'll need to submit it either by email or in person, depending on the form type.  Please note that the data you enter is directly captured in the form's unique barcode, so it's important that you do not make any handwritten changes, corrections, or additions to the data or to the form after it has been printed. Doing so may result in data errors and processing delays.

In case you notice any errors after printing your form, or if you've left a field blank, we recommend starting over and completing a new form. It's also important to remember that your information is not saved by the form filler and can no longer be accessed after you print it. Therefore, please be extra careful in reviewing your answers, as it is impossible to make any corrections after the form is printed.

Each form will take approximately ten minutes to complete. Please ensure you have the needed documents such as passport to extract information that may be required in some fields. Do not close the forms until they are ready for a download. Partially filled forms cannot be saved to be completed later. If you need further assistance, please contact. Thank you!

Information

Subject:    Request for Waiver of Requirement to be Present on Last Day of Service

1. This is in response to your request to be absent from the office on your last day of service. Staff Rule 7.01 Ending Employment, paragraph 16.01 requires the staff member be present in the office on their last day of service unless the staff member's Manager decides otherwise, with the concurrence of the HR Team Manager.

2. I have discussed your request with*

and we agree to your request  provided that:

·     You will have completed all pending work assignments

·     You will have taken care of all your own administrative requirements relating to ending employment

        (e.g. returning Bank Group equipment)

·     That you have adequate annual leave to cover the period of absence prior to your termination date.

My understanding is that your last day physically present in the office is*

with your official separation date on*

3. If you resettle outside your duty station country while you remain an employee of the World Bank, you may do so providing you accept any tax liability on your Bank salary and other payments which may arise as a result of your departure from your duty station country.  The Bank will not pay tax allowance for any taxes as a result of your departure.

4. Please remember that you may not commence work with another employer while you remain in active staff status with the World Bank Group.

5. If you hold a G4 visa, it expires on your official separation date with the Bank Group regardless of the expiration date stamped on the G4 visa or on your I-94 form, as does the visa of any of your G4 dependents and G5 employees. G4 staff and dependents have a 30-day grace period to make preparations for departure from the U.S. or to change to a different U.S. visa. (G5 visa holders have no grace period.)

If you or any of your G4 dependents leaves the U.S. within those 30 days, the G4 visa of the traveler becomes immediately invalid upon departure, and the traveler may not legally re-enter the U.S. on that G4 visa. Staff and dependent family members intending to stay in the U.S. longer than 30 days should obtain an appropriate visa prior to the end of the 30-day grace period.
